Below are several forms and documents that are required from potential work team members to complete and return. Once completed the forms can be either mailed, faxed or E-mailed to the offices of Sacred Portion Children's Outreach. Please include a $50.00 application fee to cover the adminstrative costs associated with the screening process. This is a tax-deductable donation for your tax purposes.
